How do you evacuate a plane when it is crashing over an ocean?

If the plane is truly going to crash catastrophically - then there really is no way to get out. Sorry.
If the plane is going to be able to ditch in the water in a controlled manner, then you don't want to get out before it comes to rest.
Think about it. Say the plane's forward speed is maybe 100 to150 MPH, and descending. If you could get a door open while it is in flight - which is unlikely - then you would have to choose exactly the right moment to jump from the doorway - too soon and the plane will be up too high and the fall will kill you, wait too long, and there won't be enough distance between you and the plane when it lands. You might only have a couple of seconds to decide.
Even if you could pick the precise moment, you will hit the water with a forward speed of 100 to 150 MPH, which will either rip off an arm or leg if it penetrates the surface of the water but the rest of you doesn't, or you will skip over the water like a rock. If you managed to survive, you might be unconscious, so you would drown, or have multiple broken bones, or, more likely, both.
In any case, while a plane is in flight, the doors are set up to deploy the escape chutes automatically (I think) when the doors open, so you would also have to contend with the chute flapping in the wind as you tried to get out of the door.
The escape chutes on over-water flights are also life rafts, so when the crew or passengers open the doors when the plane stops, the life raft is there and people climb into it.
And don't bother getting into the whole - "why don't passengers have parachutes?" question - it has been asked in Y!A multiple times (do a search for it) - it is not a feasible solution for many reasons.
